    Training Verification

We are able to verify training only for the following accredited programs: Craniofacial Surgery, General Surgery, Hand Surgery, OMS Head & Neck Oncology, Oral & Maxillofacial Surgery, Pediatric Surgery, Plastic Surgery, Surgical Critical Care, and Vascular Surgery residencies and fellowships.

The Department of Surgery’s Office of Surgery Education charges a $35 fee for training verifications for individuals who have been out of the program for two years or more, and a $50 fee for verifications requiring the Program Director’s signature.

To request a training verification for a resident or fellow (any physician who has participated in an accredited Department of Surgery graduate medical education program—including interns, residents, and clinical fellows—at the University of Michigan), please follow these steps:

    1. Email Maia Irwin to request a training verification.
    2. Have the trainee complete the U-M Health Authorization, Release, and Immunity form (please note: forms from other institutions will not be accepted). Form will automatically be sent to the Office of Surgery Education once submitted by trainee.
    3. If applicable, a payment request will be sent to you. While verifications will be returned in good faith to avoid delays in credentialing, please note that no additional verifications will be processed for your institution/company if there is a pending payment on file.
