Online Affiliation Verification
We offer online hospital affiliation verification for privileged providers. A letter is generated showing the dates of hospital affiliation for each provider requested.
It is your responsibility to ensure that the appropriate waiver and release have been obtained prior to accessing the information on this website.
1. Create a username and password if you have not already done so.
2. Enter the provider’s name and hit enter to search.
3. Click on the name of the provider to generate a letter.
Before faxing or mailing your request, please check our website. Microsoft Edge or Google Chrome are the preferred browsers when using this verification system.
In the event you are unable to obtain the information requested from our website, please fax a signed release with your request to Fax: (734) 936-9757 and include a copy of the message showing no results found.
Please note that our electronic credentialing database was implemented in 2001. We are unable to verify hospital privileges for any providers that terminated prior to that date.
