      We present a case of a child with apparently normal cardiac anatomy who had recurrent bronchiolitis and mild persistent hypoxemia. Persistent left superior vena cava (PLSVC) subsequently identified by chest CT. Subsequent angiogram demonstrated atypical insertion into roof of the left atrium (LA). PLSVC is usually, but not always, associated with complex congenital heart disease. PLSVC with atypical insertion should be considered in the differential of chronic hypoxemia
